I would add the oils to the alcohol because it is easier to mix this way.
Shaking is ok, but stirring is sufficient.

You should start with about 3 ounces of perfumers alcohol, add essential oils starting with your bass note. Shake after the addition of each oil. Add the water or hydrosol sparingly.
Percentages can very greatly with the kind of perfume you are making. Real Perfume is more than 22% essential oils. Eau de perfume is 15 to 22 %. Eau de toilette is 8 to 15%. Cologne is less than 5% essential oils.
